$15.00 per call from our office to your location. Please print, fill in the
changes then fax to (714) 547-1040. Changes received prior to 3 p.m. will be effective by 9 a.m. the next day. Changes received after 3 p.m. are considered as received before 3 p.m. the next business day and are treated
